Documents Required for Cashless Hospitalisation
To avail cashless hospitalisation, please ensure you bring the following documents:
For Individual Insurance Policies:
- TPA (Third Party Administrator) ID Card
- Patient’s photo identity proof (e.g., Aadhaar Card, PAN Card)
- Patient’s address proof
- All previous medical records (consultation papers, investigation reports, referral letters, if any)
- List of current medications
- Birth certificate and photograph (for children below 5 years)
For Corporate/Company Insurance Policies:
- Original credit/referral letter from the respective company
- Employee ID card
- TPA ID card
- Patient’s photo identity proof
- Patient’s address proof
- All previous medical records (consultation papers, investigation reports, referral letters, if any)
- List of current medications
- Birth certificate and photograph (for children below 5 years)
